and Billing Byrdstown Medical Center's Insurance and Billing Office will prepare and file your insurance claim forms for you based on the information that you provide to us. To process your claim, it is essential that you provide accurate and complete insurance information. Please provide us with any special forms required by your insurance company. Otherwise, your claim will be processed by standard health insurance claim forms. Always bring your current insurance cards to each visit, as this will help us keep current with your insurance coverage. Managed Care Please inform us at the time the appointment is made if you are in a PPO or HMO care plan. We participate in most major plans, and will work with you if we do not participate in your plan. If you choose not to follow the rules of your plan, we will be happy to see you, however you must sign a waiver stating that you are aware that your charges will either not be covered by your managed care plan or that they will be paid at a reduced rate. By signing this waiver, you will be agreeing to pay any unpaid charges. Byrdstown Medical Center's Insurance and Billing Manager will be happy to assist you with this. Payment Policy PPO and HMO visit co-pays are due and payable at the time of the visit. Pay rendered services is requested when you receive a statement indicating a person's visit and is due in 30 days. If you have a financial hardship, please advise us at the registration and we will be happy to make special arrangements. Payment plans, or other alternatives can be provided to assist you of hardship. Insurance payments sent directly to the patient should be forwarded to the Insurance and Billing office. We
